Key Responsibilities :

Caring for vulnerable patients in their home or other community settings to provide health promotion, disease management, care transitions and other services as needed

Work alongside emergency medical services, emergency department and primary care physicians in both the clinic and hospital setting to identify patients who could benefits from community paramedic services

Work under the license of the medical director utilizing established written protocols in conjunction with the patient's primary care physician and the patients' established care plan

Schedule is typically Monday - Friday 8am - 4pm. End time can vary between 4-6p, depending on referrals. FLEX schedule.

Licensure / Certification Qualifications :

State licensure / certification as a Community Paramedic is required in the State of WI upon hire

OR Two years of Paramedic experience & willing to obtain Community Paramedic licensure within 3 months of hire. ( We will help you obtain this!)

CPR Certification

Advanced Cardiovascular Life Support (ACLS) certificate

Must meet driving requirements and criteria acceptable to Essentia Health's insurer (non-patient transport)

State background check must provide clearance to work with vulnerable adults

Current (valid) US / State driver's License
