Must-Have : Proficiency in HomeCare HomeBase and OASIS within the last 6 months

Job Description :

• Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.
• Responsible to Clinical Manager; directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
• Conduct home visits and participate in periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Handle an average of 5-8 patients per day with varying types of cases, including Start of Care, Wound Care, and Follow-up Visits.
• Travel up to 75 miles from home clinic with mileage reimbursed at the IRS standard rate.
• Manage a minimum of 30 productivity points per week, translating to roughly 40 hours.
• Complete documentation within 24 hours of patient appointments. Ensure billing and follow-up patient care are not impacted by delayed or incomplete documentation.
• Open to traveling and on-call rotations as needed.
• Participate in weekend rotations (1 to 2 per month) as required.
• Maintain proficiency with HomeCare HomeBase and undergo retraining if not used within the last 6 months.
• Local candidates accepted at the same rate.
