We are hiring for a Care Transition Coordinator– RN . $3,000 Sign-On Bonus! Schedule is Mon-Fri, 10am-6pm, no on-call. This position has a role as a RN and also as an Admissions / Sales / Marketing representative for the agency.  Does require travel to patients in the hospital and occasional travel in the community.
