Job descriptionClinical Nurse is a Registered Professional Nurse providing safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care incorporating Relationship Centered Care and nursing process based on theory, research, evidence-based practice, and organizational nursing standards. Assesses and evaluates patient care needs applying critical thinking skills. Collaborates with interdisciplinary healthcare team in plan of care development, implementation, and evaluation. Prioritizes patient care including teaching, rounding, coaching, planning after hospital care, and delegating as appropriate. Educates patients and caregivers about plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, family, and healthcare team. Manages assignments within Modified Primary Nursing care delivery model and allocates material resources effectively. Role models Mount Sinai Relationship Centered Care through various patient care actions such as Admission Welcome, Bedside shift Report, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD / RN Unit Collaboration. Considers patient's values, preferences, cultural diversity, and expressed needs in care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patient and family concerns and escalates as needed. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff use of evidence-based practices to improve patient care understanding. Conducts Purposeful Hourly Rounding addressing the 4 Ps. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in patient population. Implements process improvement strategies based on institutional and evidence-based procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams on quality improvement projects, evidence-based practice activities, and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements high reliability principles to avoid high risk behaviors and provide a safe environment. Escalates potential safety hazards or gaps following institutional chain of command. Documents nursing practice using institutional information technology to support quality and performance improvement initiatives. NYS RN License required. BLS certification required. ACLS certification required. Minimum 2 years of ICU Med Surg experience. Minimum 2 years RN experience. 1 year travel experience required for Travel positions, not required for Per Diem positions. EPIC experience highly preferred. 3x12 hour shifts schedule. Weekends required. Unit : 7E SICU / MICU Unit Average Unit Census : 17 patients 24 Beds in Unit Staffing Guidelines : 1 : 2 nurse to patient ratio Unit : 7E SICU / MICU Unit - Mount Sinai St. Luke's Average Unit Census : 17 patients Unit has 24 beds. Staffing Guidelines : 1 nurse to 2 patients ratio. Charge nurse duties may be assigned. Coordinates with Clinical Coordinator / Manager to ensure supplies and inventory for patient care. Escalates material or staffing deficits as needed. Demonstrates patient-focused and cost-effective approaches to patient care resources. Documents accurately and thoroughly in compliance with hospital, regulatory, and legal standards. Facilitates patient throughput on shift basis. Contributes to environment of care supporting clinical colleagues, patients, families, and healthcare team. Advances clinical competence from novice to expert. Projects professional image and communicates with caring, respect, compassion, and empathy. Incorporates ethical principles into decision making for patients and families. Encourages scholarship, inquiry, lifelong learning, and innovation. Acquires knowledge and skills relevant to role, patient population, clinical specialty, and health community needs. Participates in shared decision making through specialty practice councils, nursing committees, and unit initiatives.
