Homecare Homebase charting experience is required

Minimum of 1 year of experience is required (Home Health experience is preferred)

Description :

Home Health Services are provided in various areas.

The typical caseload involves seeing an average of 6-7 patients per day, which translates to about 30 patients per week.

Admissions are weighted at 2.40 units, with approximately 2 admissions expected per day.

Patients are 18 years and older; there are no pediatric cases.

Common patient needs include wound care, wound vacs, Pleurx drains, catheter care (foley and SP), feeding tubes, IVs (peripheral and PICCs / Ports), diabetes management, heart failure, and medication and disease management.

Homecare Homebase charting experience is essential.

Required skills include knowledge of Oasis, hospice philosophy, infection control practices, wound care, PICC line dressing changes, port access, IV infusions, and drain management.

Critical thinking and catheter insertion skills are necessary.

Candidates must be able to drive at night.

Support within the department is provided by leadership, LPNs, CNAs, and peers.

Scheduling is typically Monday through Friday from 8 AM to 5 PM.

Weekends and holidays are dependent on needs and will be discussed during the interview.

Scrub color is navy.
