Job Summary :

We’re looking for an experienced and compassionate Registered Nurse (RN) to join a Long-Term Care / Skilled Nursing team in Wisconsin. The RN will provide high-quality care to adult and geriatric patients with multiple medical conditions in a skilled nursing environment.

Key Responsibilities :

Deliver direct patient care in a long-term care or rehab setting.

Administer medications, including insulin and IV therapies.

Perform wound care, dressing changes, and manage wound vacs and drains.

Monitor vital signs, glucose levels, and overall patient condition.

Provide colostomy and Foley catheter care.

Collaborate with the care team to develop and update individualized care plans.

Educate patients and families about health management and recovery.

Qualifications :

Active Registered Nurse (RN) license (Wisconsin or Compact).

Minimum 1 year of LTC or Skilled Nursing experience required.

BLS certification required.

Experience with PointClickCare charting preferred.

Strong clinical assessment and wound care skills.
