A hospital pharmacy opportunity is available for a dedicated pharmacist skilled in inpatient medication management. This contract position is based in Glenwood Springs, CO, with a schedule of 7 nights on followed by 7 nights off, ideal for those seeking a balanced work routine.

Key Responsibilities :

• Interpret and transcribe medication orders, maintaining accurate puterized patient medication profiles.
• pound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition.
• Ensure pliance with federal, state, and local regulations, maintaining controlled substance records.
• Monitor drug regimens for contraindications, interactions, allergies, and dosage appropriateness.
• Perform pharmacokinetic drug dosing and review culture and sensitivity data to remend antibiotic therapy adjustments.
• Extract and interpret patient chart information, detect and report adverse drug reactions.
• Support formulary management by minimizing non-formulary use and promoting therapeutic substitutions.
• Offer clinical consultation, provide drug information to healthcare staff, patients, and families.
• Document all clinical activities accurately and participate in quality improvement initiatives.
• Assist in staff education and training; serve as a member of the Code Response Team.
