Position Overview :

The Telephonic Case Manager RN - Medical Oncology is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the continuum of care. This includes clinical operations and medical management activities such as case management and consulting for medical management.

Work Schedule :

This is a full-time position, Monday - Friday, from 8 am to 5 pm in your time zone, with the flexibility to work remotely.

Primary Responsibilities :

  • Conduct outbound calls and receive inbound calls to evaluate members' current health status.
  • Identify and address gaps or barriers in treatment plans.
  • Offer patient education to empower self-management.
  • Facilitate referrals to external resources.
  • Ensure continuity of quality care through effective communication with members.
  • Assist members with educational resources, medication reviews, and referrals to services such as Home Health Aides or Meals on Wheels.

This role operates in a high-volume, customer service environment, requiring efficiency, productivity, and thoroughness in member interactions. Strong computer and software navigation skills are essential, along with a patient-centered and adaptable approach.

Required Qualifications :

  • Current, unrestricted RN license in state of residence.
  • Active Compact RN License or ability to acquire upon hire.
  • Minimum of 3 years of experience in a hospital, acute care, or direct care setting.
  • Proficient typing skills and experience navigating a Windows-based environment.
  • Access to high-speed internet (DSL or Cable).
  • Established dedicated workspace that ensures privacy.
  • Preferred Qualifications :

  • Bachelor of Science in Nursing (BSN).
  • Certified Case Manager (CCM).
  • 1+ years of experience in a Medical / Oncology setting.
  • Prior case management experience.
  • Familiarity with discharge planning.
  • Experience in a telephonic role.
  • Knowledge of managed care.
