Pharmacist

The Pharmacist plays a critical role in ensuring the safe, effective, and ethical delivery of pharmaceutical care to patients. Working under the general direction of the Pharmacy Director or Pharmacist-in-Charge (PIC), the Pharmacist is responsible for reviewing, preparing, and dispensing medications; providing clinical consultation to patients and healthcare professionals; and maintaining compliance with all state and federal regulations. The ideal candidate demonstrates clinical expertise, attention to detail, and a strong commitment to patient-centered care. • Ensure compliance with all California State Board of Pharmacy and federal regulations, including those related to dispensing, recordkeeping, and controlled substance management.
• Accurately review and verify prescriptions for appropriateness, dosage, drug interactions, and therapeutic compatibility prior to dispensing.
• Compound and dispense medications according to legal, ethical, and professional standards, ensuring accuracy and safety.
• Provide medication counseling to patients regarding proper use, potential side effects, drug interactions, and adherence to prescribed therapies.
• Conduct medication therapy reviews (MTM) to identify, resolve, and prevent medication-related issues and to optimize patient outcomes.
• Collaborate with physicians, nurses, and other healthcare providers to develop and implement individualized medication treatment plans.
• Monitor patient progress and report adverse drug reactions, therapeutic failures, or complications to prescribers and regulatory agencies as appropriate.
• Maintain accurate and confidential patient medication records, including documentation of interventions, therapy outcomes, and clinical recommendations.
• Participate in quality assurance and performance improvement activities, supporting continuous improvement in medication safety and pharmacy operations.
• Assist in maintaining pharmacy inventory, including verifying orders, managing stock levels, and ensuring proper storage and labeling of medications.
• Support the training and supervision of pharmacy staff, including technicians, interns, and assistants, by providing clinical guidance and oversight.
• Assist with insurance processing, billing inquiries, and reimbursement documentation to ensure timely and accurate claim submission.
• Stay current with new drugs, treatment guidelines, and therapeutic advances, incorporating evidence-based practices into patient care.
• Participate in community outreach and patient education initiatives, promoting medication safety and public health awareness.
• Adhere to HIPAA and all applicable privacy laws to protect patient information and ensure confidentiality.
• Foster a collaborative, respectful, and patient-focused environment in coordination with pharmacy staff, clinical teams, and organizational leadership.
• Perform other duties as assigned by the Pharmacist-in-Charge, Pharmacy Director, or executive leadership, consistent with professional practice standards.
• Reports any safety hazards.
• Attends all STP mandatory meetings and other meetings as requested.
• Comply with all safety, emergency, hazard, OSHA, sterilization, clinical, quality assurance, and administrative protocols, guidelines, and standards.
• Adhere to HIPAA regulations and other relevant laws to protect patient privacy and confidentiality in all communications.
• Perform other duties as assigned by the Chief Executive Officer and executive team.
