• Required for presentation : must have actual state license board certification in hand, WILL ACCEPT PENDING DEA.
• URGENT NEED!8 shifts a monthPhysicians : Board Cert not required but must have completed a 3 year residency Will accept 1 year post grad training but must be Boarded Boards Accepted ? EM, FP and IM ( except residents) ACLS,PALS & ATLS required- ABEM EXEMPT ABPS, AAPS not accepted Proof of Immunization and PPD ?see below ?? this will affect scheduling - must have PPD with 0-30 days of working. Provider needs 2 step PPD or QuantiFERON Gold Blood Test. Must have? Criminal background check is required prior to orientation for privileges. 3rd year residents are accepted - 2 years case logs are requested not requiredANNUAL VOLUME 17400
