JOB SUMMARY
The RN Care Manager will effectively facilitate care for patients with elevated risk factors and / or chronic diseases through assessment, education, self-management support and timely healthcare delivery to achieve optimal health, quality, and financial outcomes. The RN Care Manager will contribute to improved quality of care through the efficient use of resources, thereby enhancing quality, cost-effective outcomes. They will also act as an advocate for the individual's healthcare needs, and coordinate care to minimize the fragmentation of health care delivery. This position is committed to the constant pursuit of excellence in improving the health status of the community.
Professional & Technical Skills
Home Health or Case Management experience preferred
Graduate of an accredited School of Nursing.
BSN required or obtained within 6 years of employment.
Current Indiana license as a Registered Nurse
Certification in Care Management, Cardiometabolic Health, or Health Coaching preferred.
15 continuing education hours are required annually in absence of certification.
Healthcare Provider BLS certification required.
Excellent written, verbal, listening and communication skills. Communicate appropriately and clearly to staff and providers.
Promotes positive interpersonal relationships among patients, providers, and staff.
Ability to work well independently, while collaborating with other team members. Serves as a clinical resource person to providers and staff.
Ability and willingness to self-motivate, to prioritize and change processes to improve effectiveness and efficiency. Adapts to changing patient and / or organizational priorities.
