California Licensed RN Case Manager

A company is looking for an RN Case Manager - Transition of Care (Outpatient, Bilingual Spanish or Vietnamese) - Remote.

Key Responsibilities

Develop and manage personalized discharge care plans

Coordinate services across providers and community resources

Educate members and families on diagnoses, medications, and self-care

Required Qualifications

Active, unrestricted California RN license

Bilingual in Spanish or Vietnamese

2+ years of case management or clinical experience

Willingness to obtain licensure in other designated states within the first 6 months of employment

Preferred : Case Management Certification
