Job Description :

Sunbelt Staffing is seeking a dedicated Home Health RN for a contract role in New Haven, CT. This position involves managing an in-home caseload, performing skilled nursing visits, and completing timely OASIS documentation. Ideal candidates are independent, organized, and confident navigating electronic charting.

Key Responsibilities :

Conduct admissions, routine visits, and discharges in patient homes

Complete OASIS assessments in Homecare Homebase (HCHB)

Provide skilled interventions including wound care, IV administration, and medication teaching

Educate patients and caregivers on treatment plans and chronic condition management

Coordinate with physicians and the interdisciplinary care team

Qualifications :

Active RN license in Connecticut

Minimum of 2 years of RN experience, including at least 1 year in home health

Proficiency in OASIS and Homecare Homebase (HCHB) required

Current BLS certification

Reliable transportation, valid drivers license, and auto insurance

Ability to meet Joint Commission compliance standards
