Medical Documentation Specialist

We are seeking a detail-oriented and motivated Medical Documentation Specialist to join our Value-Based Medicine team. This critical role supports our providers by preparing patient charts in real time (live pre-charting) to ensure efficient, high-quality patient visits. You'll play a key role in closing care gaps and supporting value-based care initiatives across preventive screenings, chronic disease management, and medication adherence.

• Responsibilities
• Place necessary orders for labs, screenings, and referrals
• Follow standard protocols and use decision tools when needed
• Maintain patient work queue
• Experience
• Able to navigate through several tasks efficiently and quickly
• 1+ year in a medical office, care coordination, or EHR-focused role
• Familiarity with HEDIS measures or value-based care initiatives

Job Type : Full-time

Pay : $19.00 - $20.00 per hour

Expected hours : 40 per week
