Please list if PT would like to be considered for a permanent position after the assignment, or if they prefer to continue traveling, upon application.Combined outpatient meets home health PT model of care, working closely with an interdisciplinary team in the geriatric population. You will meet with our participants at the center or at their place of residence to complete PT sessions, conduct initial, 180, change of status assessments, assessment placement for any concerns based on the participant's level of care and evaluations and ordering of DMEs. You will work with families, AFHs, ALF, SNFs, and hospitals to assess and promote safe mobility for our aging patients. Work independently on participant's care plan around PT. Work closely with IDT around participants care in general. Attend morning meetings and rehab meetings and attend care conferences as needed. Work collaboratively with rehab unit at the center to prevent gaps in care. No weekends, no nights, no major holidays.3 Interview Times (Including Time Zone) must be included when submitting candidate for a position.
