Physical Therapists at BAMSI assist individuals in the day habilitation program in the Middleboro / Lakeville area . BAMSI offers nine Day Habilitation Centers located at pleasant and friendly sites throughout Massachusetts. Each location is fully handicap accessible and conveniently located near community resources. These Day Habilitation Centers provide an array of services to ensure that each person’s unique needs and goals are met. Each person served has an individually designed service plan that outlines his or her supports, needs and personal goals. The plan is structured to encourage independence, enhance social activity, and improve the quality of individuals’ lives.
RESPONSIBILITIES :
- Evaluate the physical therapy needs of person served in a site-based day habilitation program
- Perform physical therapy assessments : gait assessments, range of motion, strength and functional mobility
- Complete physical therapy service needs assessment for each person served every 2 years or if there is a significant change in status
- Make recommendations for suggested equipment designed to improve person served mobility
- Develop exercise programs and activities designed to improve person served overall functional mobility
- Provide training to staff and person served regarding the safe and effective use of adaptive equipment
- Participate in the development of the Day Habilitation Service Plan and Semi-Annual Reviews as a member of the Interdisciplinary Team
- Participate in Interdisciplinary Team meetings when appropriate
- Complete Provider Support Strategies when necessary for any objective recommendations
- Provide training to staff and person served regarding any physical therapy recommended objectives, if necessary
- Maintain collaborative working relationships with person served support network
- Attend meetings and appropriate trainings
- Perform other related duties as needed or assigned by supervisor
- Execute duties to reflect reasonable safety standards. Universal / standard precautions must be utilized and training obtained in areas that constitute risk
QUALIFICATIONS :
Massachusetts license as Physical Therapist requiredMust possess a valid drivers license in the state of residence and have reliable transportation.Ability to lift, transfer, push / pull, maneuver and reposition 25 pounds on a regular basis.Must be able to stoop, squat, sit or otherwise position self to closely interact with persons servedMust be able to bend, reach, and stretchBAMSI is a 501(c) 3 nonprofit organization founded in Brockton, Massachusetts, with a vision of driving change and creating equal opportunities for individuals with developmental disabilities and mental and behavioral health challenges.
