Position Overview
The Speech Therapist (ST) will be responsible for delivering high-quality speech, language, cognitive-communication, and swallowing therapies to patients in a home health setting. The ideal candidate will have 35 years of clinical experience and be adept at assessing and treating a diverse patient population with varying conditions. The ST will work collaboratively with physicians, nurses, and other healthcare professionals to develop and implement personalized care plans that foster independence and improve patient outcomes.
Key Responsibilities
Conduct comprehensive assessments of patients' speech, language, cognitive-communication, and swallowing abilities.
Develop and implement individualized treatment plans, tailoring therapy interventions to meet the unique needs of each patient.
Provide therapeutic interventions for patients with a variety of conditions, including speech disorders, language delays, aphasia, dysphagia, cognitive impairments, and voice disorders.
Track patient progress through regular follow-ups and assessments, adjusting treatment plans as necessary to optimize outcomes.
Collaborate with interdisciplinary team members (e.g., physicians, nurses, physical and occupational therapists) to ensure comprehensive, coordinated care.
Educate patients and caregivers on exercises, techniques, and strategies to improve speech, language, and swallowing functions at home.
Maintain accurate documentation of patient assessments, treatment plans, progress notes, and discharge summaries, adhering to regulatory and company standards.
Participate in case discussions and team meetings to evaluate patient progress and adjust care strategies.
Foster a supportive environment for patients and families, offering guidance and encouragement throughout the treatment process.
Qualifications
Masters degree in Speech-Language Pathology from an accredited program.
Active licensure as a Speech-Language Pathologist (SLP) in [state].
Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
35 years of clinical experience as a Speech-Language Pathologist, ideally with experience in home health settings.
Proficiency in assessing and treating a broad range of speech, language, cognitive, and swallowing disorders.
Strong knowledge of evidence-based practices in speech therapy and the ability to develop customized treatment plans.
Exceptional communication and interpersonal skills, with the ability to effectively engage with patients, families, and healthcare teams.

Reliable transportation and flexibility to travel to patients' homes as needed.
