Job Title : Travel Registered Nurse – Case Manager

Location : Apple Valley, CA 92307

Start Date : 10 / 06 / 2025

Contract Length : 16 weeks

Shift : Days, 5x8 (8 : 00 AM – 4 : 30 PM)

Job Requirements

  • Minimum 2 years acute care case management and discharge planning (required)
  • Prior travel nursing experience required (no first-time travelers)
  • Active RN license
  • Certifications : BLS (required)
  • Charting : Epic (required)
  • Patient ratios : Days 1 : 25; Weekends 1 : 45
  • Must have hospital experience (behavioral health experience alone does not qualify)

    • Facility & Unit Details

  • Unit : Case Management (Acute Care Hospital)
  • Caseload : Up to 25 patients daily
  • Orientation : 40 hours
  • Weekend rotation required (minimum 4 weekend shifts per 6-week schedule)
  • Patient Age Groups : Neonates, Adolescents, Adults, Geriatrics
  • Interdisciplinary Support : Physical Therapy, Social Services

    • Skills Needed

  • Assessments, care coordination, discharge planning, utilization review
  • Organize and participate in family meetings
  • Arrange DME, SNF, and ambulance transport independently
  • Experience with HMOs, IPAs, adoption / foster care, substance abuse treatment, homelessness, advance directives, POLST, and protective services
  • Knowledge of CMS, Joint Commission, DRGs, and regulatory standards
