Nurse Navigator – Part-Time | Medicine Clinic | $44–$48 / hr + Sign-On Bonus

Job Snapshot :

    • What You'll Do :

  • Serve as a subject matter expert in patient navigation.
  • Coordinate and streamline patient access to care, removing barriers and ensuring adherence to care plans.
  • Partner with care teams to identify gaps in services and proactively bridge resources.
  • Educate and guide patients and families, helping them make informed decisions about their care.
  • Uphold professional standards, regulatory requirements, and evidence-based practices.

    • What You Bring to the Table :

    Education :

  • Bachelor's degree in Nursing or accredited program required.
  • Master's in Health Administration preferred.

    • Licensure & Certifications :

  • Active RN license (Single State or Compact).
  • BLS required; BLS Instructor preferred.

    • Experience :

  • Minimum 3 years nursing practice required.
  • Minimum 2 years disease-specific nursing experience preferred.
  • Outpatient care or care coordination experience preferred.

    • Skills & Strengths :

  • Strong communication and interpersonal skills.
  • Ability to coordinate care across multiple teams and services.
  • Detail-oriented, patient-focused, and proactive problem-solver.
