This role involves collaboration with various medical professionals and delivering quality care to patients. You will also conduct the initial visit to assess the patient's eligibility for home care services and develop individualized care plans.\n\nQualifications : \n
• Graduate of an accredited nursing program (diploma, Associate, or Baccalaureate).\n
• Valid state license as a Registered Nurse.\n
• Current Driver's License.\n
• At least one year of RN experience; two years preferred.\n
• Current CPR certification and Hepatitis consent / declination proof required.\n
