North Star’s mission is to restore women from trafficking : for freedom. for life. and forever. Motivated by the love of Jesus, we work to end trafficking and to empower survivors to reclaim the lives they were created to live through healing their whole person. In addition to other programs, North Star operates a residential program in Lancaster County, andnti-trafficking program in Berks County, and a separate social enterprise, Florissant Collective.
Summary :
The Senior Case Manager provides comprehensive, individual case management to residents, applying trauma-informed principles to support residents’ goals within the framework of the program. The Senior Case Manager reports directly to the Clinical Director. This is a full-time, exempt position (salaried, 40-45 hours per week).
The Senior Case Manager must be :
- Willing to sign the North Star Initiative Statement of Faith (Code of Beliefs)
- Willing to sign the Kingdom Living policy
- A current and active member and / or regular attendee of a local church
- Able to fully support the North Star Initiative's core values and ministry philosophy.
Essential Functions :
Create and maintain intentional connections and trust with the women servedConduct a needs assessment and create a corresponding Individualized Service Plan (ISP) with residents. Coordinate resident treatment plans with clinical care team, under the direction of the Clinical DirectorSupport resident goals through coordination of job searches, medical care, legal services, budgeting, appointments, etc.Coordinate schedules with Programming Manager and Residence Manager to minimize conflictsTrack progress and complete outcome surveys with each resident, such as Assessment of Survivor Outcomes (ASO) and others assigned by the Clinical Director, reporting results to clinical care team.Meet weekly with individual residents for case managementMaintain physical and electronic individual resident files and documentation according to organization policiesAct as liaison between residents and established community resources,Actively work with Clinical Director and program team to process referrals, interviews, and new resident onboardingSupport residents’ medication management as part of the medication teamNetwork with community agencies to cultivate resources that may benefit resident goalsFacilitate referrals and resources for survivors in the community who may not qualify for an NSI program but are in need of careEnsure compliance with all mandated legal / regulatory requirements, e.g., occupational health & safety, medication management, standards of professional conduct, etc.Attend meetings, trainings, and abide by NSI policies and procedures.Be available for On Call staff and resident support, as needed (typically 1 week / month)Assist in the development of trauma-informed policies and protocolsPractice a trauma-informed and strength-based approach when working with the women.Provide updates on residents’ progress towards goals to direct care staffProvide oversight and support to case management in Berks-based progammingAdditional responsibilities as determined by program needsEducation and Experience :
Minimum :Bachelor Degree in Social Work (BSW)Awareness of the impact of human trafficking and traumaExcellent oral and written communication skillsProficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint), Microsoft products and database systemsTwo years of experience working in human servicesPreferred :Master Degree in Social Work (MSW)Experience working with the impact of human trafficking and traumaFour years of experience working in direct care, knowledge of social work ethicsQualifications :
