Overview :

The home health registered nurse plans, organizes, and directs / coordinates home care services. This role requires experience in nursing, with an emphasis on community health education / experience. As a professional nurse, you will utilize the resources of the community to plan and direct services that meet the needs of individuals and families within their homes and communities.

Duties & Responsibilities :

• Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
• Provide preventative and rehabilitative nursing procedures as ordered by the physician
• Offer patient, caregiver, and family counseling and education
• Prepare clinical notes and communicate with the physician and other healthcare practitioners
• Identify discharge planning needs and implement prior to patient discharge
• Communicate with the primary physician and community health related persons to coordinate the care plan
• Participate in quality assessment and performance improvement programs
• Supervise home health aides and evaluate their care
