Outpatient Counselor - Youth

Crossroads Health provides a continuum of quality life-changing behavioral health and specialized chemical dependency treatment services to children, adolescents, young adults, adults and families. We seek an outgoing, friendly and passionate Youth Counselor to provide individual and family therapy to our clients in Lake County. The Youth Counselor will provide therapeutic services to children, youth and their families. Services include assessment and diagnosis, individual, group and family counseling, parent guidance; information and referral and other programs to provide the highest level of care for our clients.

Responsibilities :

• Performs diagnostic assessments as necessary
• Develops client-centered ISPs, monitor and review progress
• Provides outpatient individual, group and family therapy to children, adolescents, and families
• Maintains clinical records within Crossroads' standards
• Arranges appointments and collateral contacts, and coordinates therapeutic efforts
• Participates in outcome tracking
• Provides direct service to clients in the event of an emergency, or in the absence of an assigned service provider
• Has evening hours until 9pm 2x a week
