Responsibilities :
- Provide family support services that enhance family functioning and child well-being.
- Coordinate with community providers to facilitate comprehensive care and referrals for families.
- Engage in collaborative case planning and case review meetings as required.
- Conduct case management activities including :
- Supervised visitation
- Parent aide support
- Behavioral aide assistance
- Transportation services
- Crisis intervention
- Advocacy and linkage to services
- Complete in-home visits to monitor progress and provide ongoing support for families involved with DFCS.
- Offer parenting education and model positive parenting strategies.
- Work closely with external professionals (e.g., schools, therapists) for service alignment.
- Maintain accurate documentation of services rendered per agency expectations.
- Provide transportation services related to the family service plan as appropriate.
Qualifications :
Bachelor's degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice) or a High School Diploma / GED with at least 5 years of verifiable experience in the field.Strong understanding of family systems, trauma-informed care, and community resources.Reliable transportation, valid driver's license, and proof of insurance.Willingness to complete required background checks and contractor onboarding.Deliverables :
Timely submission of service logs, progress updates, and reports.Accurate documentation adhering to billing and reporting standards.Provision of services in accordance with contract terms, including hourly or task-based invoicing.Join a team where your contributions truly make a difference in the lives of others.
