Hourly Wage : $26.94 / Regular / Full-Time / Supervisory

The Program Manager for the White Swan Clinic Program is a key leadership role responsible for managing and overseeing the delivery of health services aimed at improving the well-being of Tribal members. The Program Manager will utilize their skills and expertise to plan, implement, and evaluate health initiatives tailored to the unique cultural, social, and healthcare needs of the Tribal community. The Program Manager ensures that programs align with both Tribal priorities and state / federal health regulations.

Examples of Work Performed :

Provide strategic leadership and direction for the White Swan Clinic, ensuring that it addresses the health needs of Tribal members.

Develop, implement, and monitor health programs and services that promote wellness, disease prevention, and health education, with a focus on culturally appropriate practices.

Ensure that program goals, objectives, and outcomes align with Tribal Health Department priorities and funding requirements.

Supervise and support program staff, including community health nurses, health educators, outreach workers, and administrative staff.

Oversee the budget and financial aspects of the program, ensuring proper allocation of resources and compliance with financial regulations.

Provide direct care when necessary, including assessments, and health screenings.

Coordinate with medical providers, behavioral health specialists, and traditional healers to offer integrated, holistic care for patients.

Develop care plans for patients with chronic conditions or at high risk, ensuring culturally competent approaches are used.

Ensure that health services are delivered in accordance with nursing standards and regulations, Tribal policies, and grant requirements.

Design and implement community health initiatives, including workshops, seminars, and health fairs, to raise awareness of health risks, prevention, and treatment.

Collaborate with Tribal leadership, elders, and other community members to integrate traditional health practices and knowledge into modern health services.

Serve as a resource for health education, ensuring that the community has access to reliable information on topics such as nutrition, diabetes management, substance abuse prevention, and mental health.

Oversee data collection and reporting efforts to monitor health outcomes and program effectiveness, utilizing health data to adjust programs as needed.

Ensure compliance with reporting requirements for Tribal, federal, and state grants, including preparing timely reports and documentation.

Use data to identify health disparities, gaps in services, and areas for program improvement.

Build and maintain partnerships with local, state, and federal health agencies, as well as Tribal organizations and other stakeholders.

Advocate for Tribal health needs at the local, regional, and national levels, attending health-related conferences, boards, and coalitions.

Serve as a liaison between the Tribal Health Program and funding agencies, ensuring that the program is compliant with all grant requirements and regulations.

Ensure that the White Swan Health Clinic is in compliance with all applicable healthcare regulations, including HIPAA, OSHA, and other public health mandates.

Implement quality improvement measures and ensure that staff are trained in safe and effective clinical practices.

Maintain confidentiality and protect the rights of patients in accordance with Tribal and federal laws.

Other duties as assigned by THO.

Knowledge, Skills and Abilities :

In-depth understanding of public health practices, including preventive health care, disease control, health promotion, and community wellness strategies.

Knowledge of Native American healthcare systems, including Indian Health Service (IHS) and Tribal Health programs, as well as the unique health challenges faced by tribal communities.

Understanding of Native American culture, traditions, and values, including traditional healing practices and how

they intersect with modern health care.

Familiarity with local, state, and federal health regulations, as well as tribal sovereignty and its impact on health policy development. Knowledge of grant application processes, reporting requirements, and financial management practices, including budgeting, auditing, and compliance.

Strong leadership skills with the ability to manage multidisciplinary teams, mentor staff, and foster a collaborative and supportive work environment.

Excellent verbal and written communication skills, with the ability to communicate effectively with diverse stakeholders, including tribal leadership, community members, and external partners.

Proficiency in writing successful grant applications and producing detailed, accurate reports for funding agencies and tribal leadership.

Ability to design, implement, and evaluate health programs that are responsive to the community's needs and aligned with strategic health goals.

Comfortable presenting health information to large groups, community members, and stakeholders, and leading community engagement sessions.

Strong critical thinking and problem-solving skills, with the ability to address complex health issues and implement effective solutions.

Ability to engage effectively with tribal community members to build trust and foster participation in health initiatives.

Ability to interpret health data and use it to inform program planning, policy development, and reporting.

Ability to manage multiple projects and priorities, often within tight deadlines, while maintaining attention to detail.

Ability to incorporate cultural knowledge and respect for traditions into health programs and services, ensuring they are culturally appropriate.

Flexibility in adapting to changing health needs, community dynamics, and regulatory environments.

Minimum Requirements :

Bachelor's degree required; minimum 5 years social work or healthcare leadership experience.

Demonstrated experience in program management, including planning, budgeting, and reporting.

Must possess a current First Aide / CPR card or ability to obtain one within 30 days of employment.

Required to pass a pre-employment background check.

Required to pass a pre-employment drug test.

Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.

Preferred Requirements :

Master's degree preferred.

Experience working in Tribal health, rural health, or community-based health settings preferred.

Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
