Summary of Job Duties :

Licensed Certified Social Worker assists with patient management in an outpatient setting as part of a multidisciplinary care management team. Responsible for assessment of the psychological, financial, and other needs of the patients and families. Provides information, counseling, and referrals to other levels of care and appropriate resources for the patient.
