Hospice Registered Nurse, Full Time, Days, Essex County NJ

Job Description

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time hospice Registered Nurse to work 9 AM to 5 PM in the Essex County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require hospice care at one of our hospital sites, sometimes in their homes as needed. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.

Entry level offers start at $43.00 / hour and increase based upon experience. Mileage reimbursement ($.70) included.

Internal referral bonus : $5,000

Reports to : Assistant Manager

Duties & Responsibilities :

Performs Patient assessments.

• Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals
• Develops the plan of care for the patient and family

Ensures appropriate utilization of services

Completes discharge assessments

Uses age appropriate teaching methods for patients and families

Assumes responsibility for Case Management.

Maintains safe environment for patients and co-workers

Attends monthly staff meeting

Assumes responsibility for case management.

Participates in Performance Improvement Program

Assists with growing agency business

Qualifications for Internal Candidates

Required :

1. Graduate of an accredited Nursing Program- Associates Degree.

2. Current state licensure as a Registered Nurse.

3. BLS certification.

Preferred :

1. Bachelor's degree in nursing.

a. All ASN must complete their BSN studies within five years of their start date.

2. Master's degree in nursing or related field.

3. 1+ year of experience in Nursing or relevant area.

4. ACLS certification.

Hospice

At Atlantic Visiting Nurse, we believe that those living with advanced illnesses should spend their final days in comfort and peace. Our compassionate, specialized care is designed to minimize patient suffering and to support families and loved ones in their roles as caregivers. As one of the first hospices in New Jersey and the United States, we have more than 40 years' experience serving private homes, nursing facilities and assisted living facilities in Essex, Morris, Somerset, and Union counties. When patients and their primary care physicians agree that curative treatment is no longer appropriate or desired, a referral to Atlantic Hospice may be appropriate. Our hospice team works with referring doctors to develop individualized care plans that manage pain, control symptoms, and ensure dignity and respect in the final days of life.
