Key Responsibilities

Collaborate with clinicians, case managers, and community health workers to provide patient-centered care

Conduct psychosocial screenings and develop individualized care plans

Assist families in navigating complex support issues and facilitate access to resources

Required Qualifications

Master's degree in social work and an active license

Minimum of 3 years of experience in medical social work, case management, or care coordination

Comfort with new technologies and remote work environments
