Certified Aging in Place Specialist (CAPS) Case Manager

📍 Winter Park, FL | Serving 7 Central Florida Counties

💼 Full-Time | Non-Exempt

💲 $25$27 / hour

🚗 Company Vehicle Provided or Mileage Reimbursement

About the Role

Are you passionate about helping older adults and individuals with disabilities live safely and independently in their homes? Were seeking a Certified Aging in Place Specialist (CAPS) Case Manager to join our team and make a meaningful impact across Central Florida.

In this role, youll conduct in-home assessments to evaluate safety, accessibility, and functional needs. Youll recommend and follow up on home modificationslike grab bars, wheelchair ramps, and other adaptive solutionsto improve quality of life for our clients, most of whom are 62+ or disabled adults.

Key Responsibilities

  • Conduct comprehensive in-home assessments for ADLs and IADLs.
  • Identify fall risks, safety hazards, and accessibility challenges.
  • Recommend and document home modifications (e.g., grab bar placement, ramps).
  • Perform follow-up visits to ensure modifications are effective and safe.
  • Educate clients on equipment use and safety.
  • Inspect completed work and issue reports for corrections if needed.
  • Maintain accurate case notes and documentation in our system.
  • Travel across 7 counties in Central Florida (4050% travel).

Qualifications

  • Required : Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) license.
  • Preferred : CAPS certification (well pay for your 3-day online training if you dont have it).
  • Valid drivers license and willingness to travel.
  • Strong technical and documentation skills.
  • Compassionate, detail-oriented, and client-focused.
