Louisiana Licensed Care Management Associate

A company is looking for a Care Management Associate to manage enrollment and support comprehensive coordination of medical services for population health members.

Key Responsibilities

Manage enrollment for child and family welfare population health members and carry a low tier caseload

Coordinate and implement Wellness Plans, including community resource engagement and care gap reviews

Conduct initial reviews and triage Care Team tasks while ensuring compliance with regulatory standards

Required Qualifications

Must reside in Louisiana

2-4 years of experience in healthcare or working with foster, child and family welfare populations

High School Diploma or G.E.D. is required

Proficiency with personal computers and MS Office Suite applications

Flexibility to work occasional nights and weekends outside of standard business hours
