Summary : SJFMC is looking to hire a Licensed Professional Counselor to join our collaborative team providing services to underserved communities in New Jersey. The LCSW will be provided an initial caseload of clients to be seen weekly for therapy sessions.

Position Responsibilities / Duties :

• Responsible for assessing patients, utilizing therapeutic techniques, and providing supportive counseling to individuals and groups
• Facilitate psychoeducation and empowerment, and the sharing of experience and knowledge
• Evaluate clients' needs and develop personalized treatment plans
• Advocate on behalf of the client and make referrals for appointments and provide assistance
• Support each individual's desire for independence and increased self-confidence in client-centered environment, using evidenced-based techniques.
• Consistently document treatment session content and generate individualized and group progress notes in a timely manner
• Build and maintain positive relationships and ongoing communication with the team, staff members, patients and families
• Crisis intervention as applicable
• Provide clinical supervision as needed to LSWs, as well as guidance and support to the MHA / CMA.
• Other duties as assigned to support organizational and program goals
