Job descriptionWork–Life Balance – predictable schedules, no excessive call requirements Collaborative Culture – a respectful, team-based environment where providers support one another Ongoing Learning – monthly didactic sessions to foster clinical growth and collaboration Autonomy with Support – practice at the top of your license while knowing help is available Leadership tracks – opportunities to grow into leadership and mentorship roles Professional Development – CME allowance, mentorship, and growth opportunities Stable Practice – loyal patient base across Rancho Cucamonga, Chino, Rialto, and Riverside (opening soon) Provide comprehensive patient care in urgent care and / or primary care settings Perform histories, physicals, diagnosis, and treatment planning Order and interpret diagnostic tests Prescribe medications and manage chronic conditions within scope of practice Collaborate with supervising physicians and care teams to ensure coordinated care Educate patients on preventive health, treatment plans, and wellness strategies Document all encounters accurately in the EMR system Current California license as a Physician Assistant or Nurse Practitioner DEA license (or eligibility) Strong clinical skills with a passion for patient-centered care Ability to work both independently and collaboratively within a team New graduates welcome to apply — experienced providers preferred Spanish language skills a plus (but not required)

- Competitive hourly rate ($70-$72) for new graduates, with higher rates available for experienced APPs (based on pertinent experience, ability to practice with minimal oversight, and capacity to manage higher patient volumes)
