Here, your expertise mattersand your work changes lives.A Day in the Life : As a part-time Hospice Nurse Practitioner, youll provide medical care, symptom management, and guidance to patients and families navigating terminal illness. Youll collaborate closely with the Interdisciplinary Group (IDG), acting as a primary care provider in support of hospice plans, ensuring patients receive the highest standard of care. From home visits to patient and family consultations, no two days are the samebut each day brings the fulfillment of making a real impact.What Youll Do : Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of Northern Illinois Hospice Medical Director.Offer clinical guidance to staff and clinical supervisors to uphold top standards in hospice care.Collaborate with patients, families, and the IDG to set goals of care, focusing on pain and symptom management.Complete face-to-face assessments and other documentation to ensure hospice eligibility and compliance.Serve as primary care for hospice patients, supporting admissions and ongoing care.Work Schedule : Part-time days and hours of work are Monday and Tuesday, 8 : 15am to 4 : 45pm and Thursday and / or Friday afternoons; occasional on-call may be required for e-scribing.
