Job Type : Full-Time, Hourly

What You’ll Do
- Deliver direct nursing care under the direction of a Registered Nurse (RN) in accordance with the plan of care
- Administer medications and treatments as ordered by the physician
- Monitor and report changes in the patient's condition and ensure timely communication with the RN and interdisciplinary team
- Provide education to patients and caregivers on self-care, medications, and symptom management
- Assist with daily living activities to promote patient comfort and independence
- Maintain accurate clinical documentation and timely reporting of patient care and progress
- Participate in interdisciplinary team meetings, quality improvement initiatives, and continuing education
- Ensure safe, respectful, and compassionate care at all times
Qualifications
Licensed Practical / Vocational Nurse (LPN / LVN) in the state of employmentGraduate of an accredited Practical or Vocational Nursing programMinimum 1 year of nursing experience (hospice, home health, or related preferred)Active Basic Life Support (BLS / CPR) certificationValid driver’s license, auto insurance, and reliable transportationStrong communication and time management skillsComfortable working independently and in rural or home settingsDemonstrates compassion, empathy, and a deep understanding of end-of-life carePlease note : Legacy Hospice is a drug-free workplace.
