Certified Nurse Assistant Full Time Days - Home Care Pocono

$1500 SIGN ON BONUS!

Provides hands on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist. Provides other support services as a member of the care team.

Job Duties

• Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
• Assumes responsibility for and completes routine and complex assignments for assigned home care / hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
• Observes changes in patient functional status and personal care needs; documents observations and reports changes and / or findings to supervising clinician or clinical manager as appropriate.

Minimum Qualifications

Physical Demands

Lift over 50 lbs. Push / pull up to 140 lbs. Carry 80 lbs. short distances. Frequent standing / walking, squatting, kneeling, crawling, climbing and reaching overhead.
