Job description

Purpose :

To coordinate and provide direct services for youth admitted to Homeward Bound Shelter and the Independent Living Program.

Job Responsibilities :

  • Review referrals for program eligibility and complete intakes as needed.
  • Conduct assessments and maintain documentation of youth files.
  • Work with other Treatment Team members to develop individualized service plan.
  • Provide one-on-one case management and life skills groups addressing appropriate service goals.
  • Coordinate the medical and educational needs of Homeward Bound Residents.
  • Collaborate with DCBS, school personnel, families, and other service providers to meet needs of youth.
  • Ensure program compliance.

Requirements :

  • Bachelor's Degree.
  • 2 year's case management experience preferred, experience with adolescents desired.
  • Knowledge of best practice documentation, service plan development, and life skills assessment.
  • Residential experience is a plus.
  • Valid Driver's License with clean driving record and reliable transportation.
  • Clean background check.

  • This is an exempt position with a salary range of $36,000 - $38,000 depending on experience.
