Registered Nurse

Job Summary :

The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness within a home health setting. The role involves patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team. The RN will travel to patients within Mt. Angel, OR, and surrounding areas.

Location : Mount Angel, Oregon, United States

Responsibilities :

• Provide skilled nursing care to patients in their homes.
• Perform patient assessments and develop treatment plans.
• Administer medications and treatments as prescribed.
• Collaborate with the interdisciplinary team to ensure comprehensive patient care.
• Document patient care and related duties appropriately using EPIC.
• Manage a caseload of patients, potentially visiting up to 6 patients per day, aiming for 30 productivity points.
• Maintain accurate and timely documentation of patient visits and progress.
• Ensure compliance with all applicable regulations and standards.
