Job Summary :
The Registered Nurse provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness within a home health setting. The role involves patient assessments, medication administration, treatment planning, and collaborating with an interdisciplinary team. The RN will travel to patients within Mt. Angel, OR, and surrounding areas.

Responsibilities :
- Provide skilled nursing care to patients in their homes.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Document patient care and related duties appropriately using EPIC.
- Manage a caseload of patients, potentially visiting up to 6 patients per day, aiming for 30 productivity points.
- Maintain accurate and timely documentation of patient visits and progress.
- Ensure compliance with all applicable regulations and standards.
Required Skills & Certifications :
Active Registered Nurse license in the state of working.BCLS certification.Minimum of 1 year of skilled home health experience.Oasis experience.Driver's license, car insurance, and automobile for field work.Primary series COVID vaccination in addition to one booster.Preferred Skills & Certifications :
EPIC charting experience.Special Considerations :
Traveler is expected to have their own transportation.May need to work some weekends.Facility may call off traveler UP TO 1 shift per pay period (every two weeks).Occasional maximum radius of 50+ miles for patient visits.Scheduling :
8-hour day shifts.5 days a week.The facility cannot accommodate 4x10s or 3x12s schedules.Block schedules are not accommodated.
