Job descriptionConduct in-home assessments, including : SAI Core A to Core Q ISP (Individual Service Plan) EDP (Evaluation and Development Plan) Other applicable forms (1500 forms, SBP, EVV) Assess members??? current health status, resource utilization, treatment plans, services, and goals Follow up on requests for items / services Communicate with providers regarding care plans and services Submit assessments within 24 hours of visit and all documentation within 5 business days Complete 5 successful visits per week Active Registered Nurse (RN) license 4+ years of nursing experience Pediatric nursing experience Strong computer skills, including Excel Bilingual (Spanish preferred) Case Management experience Bachelor of Science in Nursing (BSN) preferred
A-Line Staffing is now hiring Bilingual Pediatric Care Manager (RN) in El Paso, TX. The Bilingual Pediatric Care Manager (RN) would be working for a Major Healthcare System and has career growth potential.
Bilingual Pediatric Care Manager (RN) Highlights :
- Schedule : Monday ??? Friday, 8 AM ??? 5 PM (May require after-hour visits based on member request; occasional Saturday visits)
- Pay Range : $38-$48 hourly, plus mileage reimbursement
- Location : Hybrid / Field role (Travel required within a 100+ mile radius, including surrounding counties)
