Lamoille Health Partners is seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our dynamic team. As an integral member of our patient-centered healthcare team, you will provide high-quality nursing care to a diverse population in a fast-paced community health center setting.

Responsibilities :

  • Patient Care :

Assist physicians and other healthcare providers with patient examinations and procedures.

  • Prepare patients for examinations and procedures.
  • Administer medications, including injections and vaccinations.
  • Collect and document patient vital signs and medical history.
  • Perform basic nursing assessments and triage patient calls.
  • Educate patients on health maintenance and disease prevention.
  • Assist with minor procedures, such as wound care and dressing changes.
  • Clinical Support :
  • Maintain accurate and up-to-date patient records.

  • Prepare and maintain patient charts and medical records.
  • Assist with inventory management of medical supplies.
  • Ensure compliance with infection control protocols and safety guidelines.
  • Teamwork :
  • Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team.

  • Contribute to a positive and supportive work environment.
  • Qualifications :
  • Required :
  • Current and valid LPN license in the state of Vermont

  • BLS certification
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management skills
  • Ability to work effectively in a fast-paced and diverse environment
  • Preferred :
  • Experience working in a community health center or other primary care setting

  • Experience with electronic health records (EHR) systems
  • Bilingual skills (Spanish preferred)
