Job Summary

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in patient care, with expertise in various areas of physiotherapy in a Skilled nursing facility. This role involves developing individualized care plans, providing effective treatment interventions, and ensuring optimal patient outcomes through evidence-based practices.

Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
• Develop and implement personalized care plans based on patient needs and goals.
• Provide therapeutic interventions including trigger point therapy, orthopedics, and geriatrics.
• Collaborate with interdisciplinary teams to ensure coordinated patient care across inpatient, outpatient, and home health environments.
• Educate patients and their families on treatment plans, exercises, and self-management strategies.
• Maintain accurate documentation of patient progress using EMR systems.
• Participate in discharge planning to ensure continuity of care post-treatment.
• Stay current with advancements in physiotherapy practices and medical terminology.
