What You'll Do

As a Physical Therapist, you will :

• Evaluate and Treat Patients : Modify care plans as needed to ensure optimal patient outcomes.
• Demonstrate Safe Practices : Perform patient transfers and utilize safe body mechanics to minimize injury risks.
• Design Care Plans : Create individualized plans that integrate treatment goals, outcomes, and discharge plans.
• Educate : Share your knowledge with patients, families, caregivers, and fellow healthcare providers using effective teaching methods.
• Document Care : Maintain accurate records in accordance with our Clinical Process Policy.
• Collaborate : Communicate with referring providers about patient progress and treatment recommendations.

What You Need

To thrive in this role, you should have :

Licensed Physical Therapist (PT) in Illinois.
