Job Summary :

The Outreach Team Lead is a new position that provides field-based leadership and operational support to outreach teams delivering syringe services and care coordination. This 40 hours per week, Monday through Friday, supervisory role is designed for individuals with prior leadership experience who are not current clients of Share. The Team Lead guides daily field activities, models professional boundaries, and ensures that outreach efforts are consistent, safe, and aligned with program goals. This role does include responsibilities such as coaching, documenting, etc.

Essential Functions :

Supervision and Staff Engagement – 25%

  • Provide supervision of the Outreach Team including task assignments, meeting facilitation, coaching, and documenting as needed or required.
  • Assume the duties of the Community Wellness Senior Director in their absence (vacation, illness, etc.) until they return.
  • Provide on-site leadership and peer support to Outreach Care Coordinators and volunteers.

Field Supervision – 50%

  • Work alongside staff supervised – accounts for 40% of field supervision.
  • Support crisis response and de-escalation in the field when needed.
  • Ensure all field activities reflect the mission and values of Share's program.
  • Adapt to changing field conditions including weather, client needs, and crisis situations.
  • Administrative Support – 25%

  • Coordinate daily outreach operations and ensure teams are equipped and deployed effectively.
  • Ensure adherence to protocols including syringe exchange, harm reduction practices, and safety procedures.
  • Assist in managing supplies, tracking distribution, and maintaining accurate field logs.
  • Serve as a liaison between outreach staff and program leadership, communicating updates and field needs.
  • Model trauma-informed engagement and maintain strong professional boundaries with participants.
  • Promote collaboration and problem-solving with community members, local businesses, and external partners including public health and law enforcement.
  • Assist with onboarding and orientation of new outreach staff.
  • Knowledge , Skills, and Abilities

  • Knowledge of harm reduction, and trauma informed care
  • Knowledge of challenges faced by those living in homelessness
  • Ability to be aware of and respectful towards cultural differences and health disparities
  • Skilled in crisis intervention techniques
  • Knowledge of assessment of social determinants of health and the ability to develop person-centered goals
  • Ability to build positive and trusting relationships with diverse populations, particularly people experiencing serious and persistent mental illness and substance use disorder
  • Ability to establish and maintain professional working relationships
  • Knowledge of effective time management techniques and ability to prioritize tasks.
  • Ability to provide attention to detail in documentation and record-keeping.
  • Ability to manage multiple clients and programs simultaneously.
  • Knowledge of basic computer skills, including familiarity with Microsoft Office Suite or Google Workspace.
  • Ability to effectively use virtual communication tools (Zoom, Teams, etc.).
  • Qualifications :

  • Previous experience in a leadership or team coordination role required.
  • Knowledge of harm reduction, trauma-informed care, and challenges faced by unhoused populations.
  • Strong interpersonal and communication skills.
  • Ability to maintain professional boundaries and work respectfully with individuals experiencing homelessness and substance use.
  • Physically able to work in outdoor environments including streets, woods, and encampments.
  • Proficient in using digital tools such as mobile apps, spreadsheets, and reporting platforms to support field coordination and data tracking.
  • Advanced documentation and data entry skills.
  • Valid driver's license required.
  • Preferred Qualifications :

  • Experience with data entry and using electronic health record (EHR) systems is preferred.
  • Knowledge of HIPAA and confidentiality practices preferred.
