Experience must include at least 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical dischargeEPIC CHARTING experience is REQUIRED5x8hr shifts non-negotiableGeneral Certification Requirements (BLS, etc.)BLS, RN Case Manager Other Certifications / Specialty certifications requirements? Criteria has a limitation on financial assistance for only those uninsured or underinsured. UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.CM Manager / SupervisorDepartment does not have a CM Manager at this time. Will directly report to the Social Work Manager. Chief of Care Coordination is also providing department oversight as needed.
