About the Role

We are seeking a caring and skilled Licensed Practical Nurse (LPN) to join our dedicated nursing team. In this role, you will work under the guidance of Registered Nurses and collaborate with medical providers to provide personalized, high-quality healthcare. Your contributions will be essential in delivering care, educating patients, and advocating for their needs across our five community sites.

Your Responsibilities

• Prepare patients for their visits, ensuring a smooth and efficient clinical workflow.
• Assist physicians and mid-level providers during medical procedures.
• Administer medications, vaccines, and treatments per provider orders.
• Monitor patient conditions and promptly communicate changes to RNs or providers.
• Engage in patient education, counseling, and distribute relevant health resources.
• Coordinate and follow up on internal and external referrals for specialty care.
• Document all care activities accurately in the Electronic Medical Record (EMR).
• Support daily pre-visit planning alongside Patient Care Associates.
• Maintain the inventory of clinical supplies and ensure secure medication storage.
• Conduct routine screening tests and assist with patient procedures.
• Adhere to infection control and HIPAA compliance standards.
• Flexibly float between BMS sites as needed to meet team and patient needs.

Qualifications

Graduate of an accredited Practical Nursing program.

Current NYS License, Certification, and Registration as an LPN.

A minimum of one year of experience in a clinical nursing role is preferred.

BCLS Certification and completion of Infection Control & Child Abuse training are required.

Bilingual in Spanish or Creole is a plus.

Ability to adapt and float across various BMS locations.
