• Conduct comprehensive clinical assessments to determine hospice eligibility\n
• Educate patients and families about hospice services and Medicare benefits\n
• Collaborate with physicians, case managers, and interdisciplinary teams\n
• Provide emotional support and clinical guidance during the decision-making process\n
• Initiate and complete the admission process, including documentation and coordination\n\nWhat We're Looking For\n
• Registered Nurse (RN) with a valid state license\n
• Minimum 2 years of nursing experience (hospice, palliative, ER, ICU, home health or case management preferred)\n
• Strong clinical assessment and critical thinking skills\n
• Compassionate communicator with excellent interpersonal skills\n
• Active listener who builds trust and rapport\n
• Confident in guiding others through uncertainty, addressing concerns with empathy\n
• Reliable professional with strong follow-through and autonomy\n
