Case Manager Position Summary :

As a Case Manager, you will play a crucial role in monitoring and managing patient care to ensure :

Continuity of care and optimal patient outcomes

High levels of patient satisfaction

Cost efficiency and compliance

Your responsibilities will include :

Consulting with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of care.

Monitoring patient and family satisfaction, addressing questions and concerns.

Participating in discharge planning and coordinating community resources.

Performing chart reviews to identify potential issues with service delivery and patient outcomes.

Minimum Qualifications :

Current RN licensure to practice in Kentucky

ACM (Accredited Case Manager) certification to be obtained within 2 years of hire / transfer

Associates Degree in Nursing (required)

Bachelor's Degree in Nursing (preferred)

Basic Life Support (BLS) certification to be obtained within 30 days
