POSITION TITLE : Child and Adolescent Services Case Manager

WORKSITE LOCATION : Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904

STARTING PAY : $ 18.47 EEOC CODE : Professional FLSA STATUS : Non-Exempt

SHIFT : Fulltime; Monday thru Friday, 8 : 00am - 5 : 00pm

REPORTS TO : Team Lead, Child and Adolescent Services

LEVEL OF SUPERVISION REQUIRED : General

The mission of Gulf Bend Center (GBC) is to improve the quality of life in our community for individuals and their families by providing excellent and trusted care for wellness. GBC is committed to providing quality person-centered, trauma-informed care to individuals experiencing serious mental illness, developmental disabilities, and co-occurring disorders. GBC programs focus on the issues that matter most in the lives of the children, families, and adults we serve.

JOB PURPOSE :

Provides case management, skills training, and medication training to children / adolescents with a serious emotional disturbance. Provides services to the primary caregiver or LAR. Services will be provided in the school, community and / or clinic. Is responsible for completing assessments, treatment planning, effective coordination of appropriate community services and crisis intervention. Acts as liaison between clients / families and service providers. Requires travel within the 7 counties. Meets multiple deadlines and reporting requirements. Work is performed under the general supervision of the Team Lead, Child and Adolescent Services with considerable latitude for the exercise of discretion and independent judgment.

ESSENTIAL FUNCTIONS and DUTIES :

  • 1. Completes TRR assessment and Recovery Plan in accordance with Center policy and HHSC Contract as verified by supervisory review.
  • 2. Formulates a Recovery plan to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer / families and based upon the TRR assessment as verified by supervisory review.
  • 3. Provides authorized services to consumers according to the treatment plan developed using independent clinical judgment to assess client need per supervisor review.
  • 4. Provides ninety-five percent (95%) of psychosocial rehab services in the schools and / or consumers' natural environment per supervisor review.
  • 5. Links customers and families to appropriate community resources as needed to meet their needs
  • 6. Provides customers Patient and Family Education in accordance with TRR Fidelity per supervisory review.
  • 7. Provides case management, skills training, and medication training in accordance with Medicaid Rule, HHSC Contract, and TRR Fidelity.
  • 8. Demonstrates appropriate responsiveness to consumer needs as demonstrated by no legitimate consumer, collateral or co-worker complaints.
  • 9. Submits treatment recommendations through Utilization Review based on clinical necessity per supervisor review.
  • 10. Attends and participates in all required team meetings as scheduled by supervisor unless excused by supervisor.
  • 11. Demonstrates efficient time management as evidenced by meeting all job expectations, including but not limited to meeting productivity requirement, completing documentation timely, and keeping trainings current.
  • Submits all documentation in electronic healthcare record (EHR) within two (2) business days. This includes but is not limited to documenting case management / skills training services, telephone contacts, and all other correspondence to maintain the case records in accordance with agency and regulatory standards and requirements.
  • Meets the required productivity of 80 hours of face-to-face contact each month.
  • Maintains 100% completion rate on all computer-based and other required training.
  • 15. Documents services collaboratively according to Medicaid Rule and HHSC Contract per supervisory review.
  • 16. Completes all paperwork according to Medicaid Rule, HHSC Contract and Center Policy.
  • Maintains lines of communication, when applicable, to customers to help provide quality of service.
  • Meets with new intakes within 7 days to provide case management services.
  • Completes Child and Adolescent Needs and Strengths (CANS) Assessment at least two weeks before expiration to meet contract measurement.
  • Updates all consent forms with customer / LAR before expiration.
  • Avoids expiration of diagnosis of all assigned customers to circumvent lapses in service.
  • Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
  • Performs other duties as assigned by supervisor which are consistent with the position and in compliance with agency policies and procedures.
  • Helps other team members to promote a supportive, constructive, and solution-focused environment.
  • 25. Follows fleet policy including pre- and post- inspections. Reports all fleet problems directly to supervisor within 24 hours of identifying the problem.
  • Maintains regular and reliable attendance.
  • Complies with telework / remote agreement.
  • Uses the trauma informed approach when providing services and working with others.
  • Understands the Three E's of trauma-informed care.
  • Understands the Four R's of trauma-informed care.
  • Understands the six key principles of trauma-informed care.

KNOWLEDGE, SKILLS, ABILITIES, and COMPETENCY :

General Competencies :

  • Strong written and verbal communication skills to communicate clearly and effectively, both orally and in writing and through email and phone, with strong interpersonal skills.
  • Ability to solve problems and resolve issues.
  • Must have good organizational skills to achieve multiple deadlines.
  • Ability to operate a computer, office equipment and office software to include Microsoft Word, Excel (
  • level), Outlook, Teams, Zoom. Ability to type 40 wpm.
  • Knowledge of child development and mental health.
  • Ability to engage children and families in treatment.
  • Accomplishes 95% of assigned tasks with minimal supervision.
  • Ability to work independently and meet deadlines with minimal supervision.
  • Uses sound judgment in the release of information as evidenced by lack of complaints or consumer rights findings.
  • Appropriate use of a fleet car, reporting all fleet problems directly to supervisor within 24 hours of identifying problem.
  • Trauma-informed :

    A trauma-informed approach to care "realizes the widespread impact of trauma and understands potential paths for recovery; recognizes the signs and symptoms of trauma in clients, families, staff, and others involved in the system; and responds by fully integrating knowledge about trauma into policies, procedures, and practices, and seeks to actively resist re-traumatization." The six key principles of a trauma-informed approach include : safety; trustworthiness and transparency; peer support; collaboration and mutuality; empowerment, voice and choice; and cultural, historical and gender issues (Substance Abuse and Mental Health Services Administration [2014]).

  • The Three E's of Trauma : Events, Experiences, and Effects.
  • The Four R's : Key Assumptions in a Trauma Informed Approach - Realization, Recognize, Respond, Resist Re-traumatization.
  • EQUIPMENT, TOOLS AND MATERIALS

  • Computer, telephone, cell phone, projectors, calculator, fax machine, printer, copy machine are used in performance of essential job duties.
  • Various forms of paperwork and computer programs are handled in performance of essential job duties.
  • EDUCATION AND EXPERIENCE REQUIRED :

    Required : A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are : psychology; social work; medicine; nursing; rehabilitation; counseling; sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.)

    Preferred : At least six months experience in providing mental health services to adults, children and adolescents.

    DEGREE AND / OR CERTIFICATION REQUIRED :

  • BA / BS in field specified above or licensed as a Registered Nurse in the State of Texas.

  • OTHER REQUIREMENTS OR CONDITIONS :

  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Must maintain personal liability insurance.
  • Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  • Safe driving record as defined by Gulf Bend Center.
  • Must complete all training requirements prior to the due date. Attend required trainings and meetings. Computer-based training and any other training required for the role is 100% compliant.
  • Must maintain regular attendance and reliability which is critical to business operations. This is a fulltime position. Days and hours of work are Monday through Friday, 8 : 00am to 5 : 00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of program's processes. Evening and weekend work may be required as job duties demand.
