Assessment and Planning
- Assess patients upon referral by physician (i.e. speech and communication function cognition swallowing etc.). Routinely checks patients ID bracelet prior to assessment.
- Formulates short- and long-term goals written in terms of functional outcomes with an established time frame for accomplishment.
- Involves patient and / or patients significant other in the formulation of goals and treatments plans. Plans a comprehensive treatment program which will be most beneficial to patient in reaching desired goals.
- Discusses assessment results and plans with appropriate physicians nurses and other department personnel; suggests involvement of Social Services Dietitian and other ancillary services as deemed necessary. Contacts Social Services for assistance needed (ie. securing equipment home health etc.). Notifies attending physician of significant changes in patients status or regarding matters causing concern. Documents in patients progress notes in chart or electronic medical record regarding assessment plan treatment instruction evaluation and recommendations.
- Identifies and communicates patients discharge needs at the time of the initial assessment. Demonstrates the ability to effectively communicate with patients / patients significant other and interpret verbal / nonverbal behavior.

Ensures the treatments participating in and supervising are completely and correctly performed in accordance with established policies and procedures. Ensures the safety of patients at all times. Informs and involves the Physical Therapy Director as needed. Demonstrates ability to anticipate patients needs and to care for allotted number patients within a designated time frame.
