As a community health nurse LPN , youll join a supportive interdisciplinary home health team of RNs, PTs, LNAs, SLPs, Social Workers, and administrative staff 🤝, delivering personalized care to patients in their homes while growing professionally through mentoring and ongoing education 📚.

What Youll Do

Provide skilled nursing care in home health settings 🩺

Support patients to remain safely in their homes while promoting independence and wellness 🌟

Collaborate with an interdisciplinary team to deliver comprehensive care

Travel within your assigned territory, with federally reimbursed mileage 🚗💨

Qualifications

Completion of an accredited LPN program

Current LPN licensure or compact licensure recognized in Vermont

2+ years of home health, public health, or hospice experience preferred

Previous home care experience is a plus, but not required mentoring provided 🌱

Essential Physical Requirements 💪

Ability to walk, stand, bend, lift, and move intermittently during shifts

Lift, push, or pull up to 50 lbs safely

Navigate stairs, maintain balance, kneel, crouch, twist, and reach

Manual dexterity and visual / auditory acuity for patient care and equipment use 👀🎧
