As a Licensed Practical Nurse (LPN), you will be responsible for providing compassionate and personalized care to clients needing assistance with daily living activities, ensuring their comfort, safety, and well-being. Working under the supervisor of the RN assigned to the case, Clinical Director, or Director of Operations, you will support clients by assessing their functional status in the areas of social, mental, and physical health, economics, and activities of daily living. Services provided will align with the Plan of Care established at the initial assessment of the patient and at any time if you identify a change in status or problems within the home, this needs to be reported to the Director of Operations, Clinical Director and if needed reported to the patients Case Manager and Primary Care Physician.
